Case Management Director oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Being a Case Management Director evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. What does this position look like~ A Case Manager is responsible for developing and implementing individual recovery plans. Provides interventions that decrease barriers to service. Collaborates with other treatment providers. Documents observations and information regarding services delivered in progress notes. Assists in the development of social skills and daily living skills. Promotes consumer usage of and facilitates direct access to needed community services. Promotes independence in travel, fiscal management, health care, etc. Provides assertive community outreach. Participates in team meetings and case reviews. Maintain a caseload of ICMS clients.
