Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified.     Description

    The didgwálic Wellness Center is seeking a Case Manager to join our integrated care clinic that combines primary care, behavioral health, and opiate treatment all in place.     What You'll do

    • Manage case assignments, develop service plans, track progress, and executes case closures.
    • Conduct screening assessment to determine client’s program eligibility and conduct functional assessments to determine their eligibility for other services.
    • Interview clients to determine their needs and determine which services and assistance they require.
    • Educate clients about their rights and benefit options, help guide them in the application process and provide support and documentation assistance.
    • Understand individual client care plans and connect their needs with the existing care plans and/or recommend needs to the SUD and/or Mental Health Counselor.
    • Identify and assess housing options, including emergency shelters, transitional housing, and permanent housing programs.
    • Assist clients in navigating Social Security Income (SSI) and Social Security Disability Insurance (SSDI) application process.
    • Help with the application process for age related benefits and benefits for the blind and disabled.
    • Partner up with the Northwest Justice Project Attorney’s, help the clients navigate the court system and assist them in regaining driving license privileges.
    • Liaise with relevant court personnel to advocate for clients and facilitate the resolution of legal issues.
    • Assist clients in accessing financial resources and educating them on debt management.
    • Maintain strong relationships with community organizations throughout Skagit, Whatcom, and Island Counties, government agencies, and service providers to coordinate resources and support the clients.
    • Document and maintain accurate and up to date EHR records of client interactions, service provided, and outcome/goals created and achieved.
    • Prepare reports and data to measure program and care plan effectiveness.
    • Perform all other duties as assigned by the Director of Social Work.

    Qualifications


    Must Haves: 

    • Knowledge of the skills to teach and engage in basic problem-solving strategies to support individual clients across diverse range of service users, including diversity of age, gender and gender identity, race, religion, sexual orientations, physical and mental ability, and more.
    • Recognizes the importance of individual differences that influence client behavior and applies this understanding to clinical practice.
    • Ability to maintain strict confidentiality of Behavioral Health Records and adhere to the standards for health recordkeeping, 42 CFR Part 2, HIPAA, and Privacy Act requirements.
    • Ability to work in a cross-culture environment, understands the social and cultural context of the clients at the Swinomish Wellness Program, Transitional Living Program and didgalic Wellness Center. Understands the role of trauma, historical, community, family, and personal experience in wellness and recovery.
    • Effective communication; written, verbal, and interpersonal skills.
    • Proficient with Microsoft Office Suite and other related programs.
    • Bachelor’s degree in social work (BSW), sociology, psychology, counseling, or a related discipline preferred.
    • Certified case manager (CCM) and/or accredited case manager (ACM) certification.
    • Three (3) or more years of experience in case management.
    • Prolonged periods of sitting at a desk and working on a computer.
    • Must be able to lift up to 15 pounds at times.
    • Ability to pass an extensive background check and drug screening. 
    • Tribal Affiliation a plus!
