Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified. Position qualifications:
Bachelor’s degree required; preferably in human services or related field. Bachelor’s degree in a non-related field may perform the functions of this position only if they have one year experience in a healthcare field. Previous experience with substance abuse and mental health-related case management and treatment preferred. Minimum of two (2) years human service related experience preferred. Current valid Colorado Driver’s License required. Certified Addiction Counselor II or III is strongly preferred
Job purpose:
Utilizes a variety of treatment approaches to work with a diverse population. Provides therapeutic and case management services, specifically discharge planning and aftercare of patient hospitalization. Patient population could include children, adults, individuals, couples, and groups.

Employee must occasionally lift and/or move up to 25 pounds. Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods of time at a computer. Employee must be able to travel between Mind Springs sites. May be exposed to stressful situations that involve enraged, distraught and/or intoxicated individuals with possible mental health conditions. Possible exposure to communicable disease.
