Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified.     Position Summary:
    Under the supervision and guidance of Nursing Administration, the Nurse Case Manager is responsible for the collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality cost-effective outcomes. Nurse Case Managers will work in other clinical areas as part of their job in order to maintain nursing skills and competencies and as needed based on organization needs.

    Knowledge/Skills/Abilities:
    Demonstrates good organizational skills. Provides safe and appropriate patient care within the Values & Standards of Nursing Practice at SSMH. Displays appropriate judgment and problem solving skills. Communicates positively and professionally with all internal and external customers.

    Degree of Supervision Required:
    Under supervision of the Director of Nursing or Designee.

    Licensure/Certification/Registration:
    • Current Registered Nurse License (any state), Alaska Registered Nurse License required within six (6) months of hire.
    • Current BLS Certification
    • Current ACLS Certified (for LPN, ACLS preferred)
    • PALS, NRP, TNCC, ENPC Preferred
    • Case Management Certification Preferred
    Education:
    Graduate of an Accredited School of Nursing

    Experience:
    Able to articulate and demonstrate appropriate nursing theory and practice. Three (3) to five (5) years Emergency experience preferred.

    Orientation Period:
    Based on experience and continuous assessment of progress by Nursing Administration. Orientation includes formal nursing department orientation, minimum of 2-4 days during the first two (2) weeks of employment.
