Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified. Primary Purpose of Position: Life Opportunities Therapeutic Home Services is committed to providing quality Foster Care Agency services in accordance with the rules and regulations of North Carolina regarding children up to 21 years of age. Life Opportunities Therapeutic Home Services promotes structured living environments, therapeutic interventions, and supervision to the consumers residing in a home supervised by Life Opportunities Therapeutic Home Services.
The agency shall give foster parents assistance, training, consultation, and emotional support in caring for children and in resolving problems related to their role as foster parents. Foster parents shall have one face-to-face contact per month by the social worker or case manager unless the out-of-home family services agreement or person-centered plan indicates a different schedule of face-to-face contacts for each foster child placed in the home. Phone support and 24-hour on-call support shall be provided to foster parents. Therapeutic foster care parents shall have at least 60 minutes of supervision by a qualified professional as defined in 10A NCAC 27G .0104 on a weekly basis for each therapeutic foster child placed in the foster home. Therapeutic Foster Parents providing treatment to children/youth with substance abuse treatment needs shall receive supervision from a qualified substance abuse professional as defined in 10A NCAC 27G .0104. The agency shall provide each foster parent with a Foster Parent Handbook that outlines agency procedures, requirements and expectations.

DESCRIPTION OF RESPONSIBILITES AND DUTIES:
75% Administrative Services
· Monitor, record and report symptoms and changes in client conditions.
· Consult and coordinate with health care team members to assess plan and implement a plan of care for clients residing in licensed homes monitored by this agency.
25% Organizational Supports
· Attend and participate in scheduled administrative staff meetings
· Attend and participate in clinical training, in-service, workshops and training conferences.
· Serve on and co-facilitate internal agency, committees, such as tasks forces, Quality Assurance/ Quality Improvement (QA/QI), and other committees.
· Maintain and enhanced clinical skills through professional readings and attendance at recommended training events.
