There are openings in Adult Case Management and Children's Case Management. On the Children's team Case Managers help in applying for and coordinating services for children by working directly with their parents/family.

Our work weeks are 36 hours Monday through Thursday.

JOB SUMMARY:

The Case Manager will work to enhance the quality of life for people with intellectual and developmental disabilities by assisting eligible people who have been given a funded resource to enroll into services, to maintain quality services that meet their identified needs and to monitor the services provided to assure they continue to meet the needs. The case manager will encourage and assist the people supported to connect with generic community services and develop and maintain natural supports.

The Case Manager provides the single point of contact for persons interested in receiving services through the Division for Intellectual and Developmental Disabilities in Colorado. Case Managers help people with intellectual and developmental disabilities and their families to navigate all of the different types of services that may be available to meet the person’s needs. The Case Manager must work on behalf of the individual to protect their rights and interests. Case Managers provide a variety of case management services, including but not limited to determining eligibility for services: describing services and instructing on how to apply; helping to determine the needs of the eligible individual; working together with the individual and others to develop a Service Plan; identifying other programs and services outside of the developmental disabilities system that may be of help to individuals placed on waiting lists for services (or to augment services being provided): and providing ongoing monitoring and support of services being provided.
