Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews.     Summary

    Case Manager at Sunny Glen Children’s Home is responsible for the safe release of Unaccompanied Children. The case Manager is required to maintain accurate documentation on case files and compliance with ORR Policy and Procedures, and established state and federal standards.

    Essential Duties and Responsibilities

    • Maintain a caseload in accordance with agency policies and procedures and licensing standards.
    • Conduct initial intake of children to gather sponsorship/family information.
    • Maintain weekly communication/interviews of family members or friends to help establish relationships as required by ORR for safe reunification.
    • Assist with identifying delays and/or gaps in order to help facilitate a timely release.
    • Engage Clinicians in the casework process by providing ongoing monitoring of complex cases.
    • Conduct case reviews prior to release submission on a case-by-case basis.
    • Adhere to all timeframes & deadlines as perState, ORR andInternalPolicies andProcedures
    • Coordinate all referrals/recommendations, service planning, and client documentation.

    • Engage and involve unaccompanied children and their sponsors in the casework process.
    • Provide weekly face-to-face sessions with Unaccompanied children by providing case updates or family/friend communication via phone.
    • Monitors child’s services, ensures all required services are delivered, sets initial appointments, and communicates appointments with appropriate staff as well as sponsors and family in the home country.
    • Ensure proper completion of assessments and deadlines as required per Internal and ORR Policy and Procedures
    • Coordinate referrals, service planning, and client documentation of services provided (case notes, admission assessments, incident reports, progress notes, etc.)
    • Maintains a therapeutic, and trauma-informed view of each child in his/her caseload and helps staff develop this perspective.
    • Coordinates placement disruptions and does discharge/transfer paperwork.
    • Demonstrating client progress toward risk reduction, achievement of treatment goals and positive case outcomes
    • In consultation with his/her supervisor, make decisions affecting the safety of the child.
    • Prepare, compile, and submit accurate and timely case files in case management software to continually assess ongoing changes in behavior, circumstances, or conditions that may affect child safety.
    • Complete alongside with the Clinical and Education team appropriate discharge planning prior to the Child’s release
    • Participate in weekly case staffing with assigned supervisor, case aides, and contracted staff.
    • Act as a liaison with stakeholders, including legal providers and immigration court
    • Work evenings, weekend rotations, and holidays as needed to assist with campus emergencies, crises, or ongoing reunification processes as requested by the supervisor.
    • Maintain confidentiality in all areas.
    • Attending training and meetings as scheduled.
    • Participate in weekly staff/team meetings and training.
    • Must abide by Sunny Glen Children’s Home mission of giving nurturing, trauma-informed care to children from difficult backgrounds

    Education and/or Experience

    A bachelor’s degree in social work or other human services field from an accredited college or university

    Child welfare and/or case management experience is strongly encouraged.

Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, and the general public.

    Mathematical Skills

    Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

    Certificates, Licenses, Registrations

    Valid Texas Driver's License.

    Have a record of tuberculosis screening showing the employee is free of contagious TB.
