Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified. - Willingness to travel or drive throughout the area to visit, perform quality checks, and provide services as needed.
- Must be a self-motivated and independent worker who can manage a flexible schedule.
SUMMARY OF RESPONSIBILITIES:- Conduct intakes: meet with families and/or individuals in an effort to explain the organization's services.
- Provide ongoing supervision and training to unlicensed direct care staff.
- Assign appropriately skilled Direct Support Professionals (DSPs) to ensure services are being provided at the highest quality.
- Attend quarterly, yearly, or as-needed meetings for all clients on caseload.
- Review staff monthly and quarterly notes for completion, and update as necessary.
- Create and maintain clinically appropriate goals.
- Work with families and internal supportive teams to initiate, coordinate, and facilitate, smooth positive transitions when individuals enter the program.
- Work in partnership with colleagues, families, and individuals to establish positive, trusting relationships, and consistent clear communication.
- Utilize effective problem solving skills and act as a model/mentor for others in demonstrating professional ethics, standards, and practices.
- Supervise unlicensed staff.
