Clinic RN Care Coordinator / Patient Care Coordinator (RN) – Gastrointestinal / Digestive Health / Gastrointestinal Medicine The OHSU Digestive Health Center (DHC) is a multi-disciplinary center offering services in the ambulatory setting for specialty surgical services, gastroenterology, hepatology, bariatrics and nutrition. A key program within the DHC is in the GI section / therapeutics. Within a team approach, the Patient Care Coordinator (RN) will provide care coordination, assessment and triage, medication management and clinical support to the therapeutic team in the clinic setting. Therapeutics is a specialized field where the nurse gets to know many of the patients closely and plays a key role in patient education and support. The practice requires management of care for patients that have complex gastrointestinal diseases, many of whom have oncological diagnosis and/or receiving treatment for chronic pancreatitis. Responsibilities include the care coordination and case management of individuals in the outpatient clinic. Responsible for coordinating comprehensive care for patients and will be a key contributor to ensuring that patients are guided through all stages of their treatment at OHSU. This patient population can be labor intensive given their complex diagnoses and multi-disciplinary care. This translates to increasingly complex care coordination. Patient Care Coordinator (RN) will be the core of a multi-disciplinary patient care team, which includes physicians, APP, RN, MA, and PAS-R support. This position serves as a coordinator for patients undergoing sequenced multidisciplinary treatment, and at times patients who travel from long distances from home. Responsibilities include but not limited to care coordination, triage, patient education, medication management, discharge and admit planning, and clinical support for the GI clinic faculty providers. This position will also participation in quality improvement, medical record review and will follow existing protocols determining appropriate patient scheduling.

Current, unencumbered Oregon State Registered Nurse License (RN). Bachelor's of Science in Nursing (BSN) - OR- Associate Degree in Nursing (required to enroll in BSN program within 3 years & complete within 5 years of hire). Outpatient EPIC experience (in-basket, mychart) BLS certification (MUST be through American Heart Association & must not expire during orientation). Must have demonstrated regular and consistent attendance at work as reported by past employer.
