Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified. Case Management Duties- A case manager is a professional who connects patients with health care providers, coordinates appointments and treatment plans, and helps patients meet their optimum level of health
- Use term “billable service” as a measuring stick in most cases for services
- Billable services include:
- Assisting in scheduling appointments
- Assisting in ordering refill on medications
- Assisting in applying for services through a program such as DSS, including helping fill out paperwork, making any phone calls with resident who does not feel they can handle the process on their own, or even transporting resident to the DSS office for an appointment
- Assisting residents who cannot retrieve transportation regularly to appointments they may have, with a weekly schedule in the office of appointments that residents have that need transportation to appointments
- Meeting weekly, bi-weekly, or monthly with residents on a case by case basis, where they discuss goals that the resident has within a certain time decided by both parties to work on and case manager will assist with anything that they may need help with
- Filling out bi-annual (6 month) Care plans with resident that highlights short and long term goals that are aspired to be achieved by resident while he is staying at the residency
- Filling out monthly service plans for resident and documentation for Director and state of discussions during meetings, highlighting the goals that were worked on with resident, or any goals that were added or achieved during the month
- Assisting resident with any duties that they cannot do by themselves, such as doing their laundry or cleaning their room, while also slowly teaching them ways to do it themselves that may not be overwhelming and can be learned by the resident
- Billable services include:
- Supervise who comes in and out of building and watching cameras, while making sure visitors sign in at the front desk and are gone by 10pm
- Open doors for residents or visitors when they buzz in through side or front doors, make sure visitors are known for the reason they are there and watched
- Make note of any items needed for front desk area that is running low, for example pens, sticky notes, paper clips, etc.
- Collect rent money when brought by residents, give any checks to Director, and write receipts for residents and place money into the safe underneath the front desk
- Hourly rounds to ensure the safety of the building

Schenectady (/skəˈnɛktədi/) is a city in Schenectady County, New York, United States, of which it is the county seat. As of the 2010 census, the city had a population of 66,135.
