Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified.     JOB SUMMARY:

    Volunteers of America of North Louisiana is seeking to hire a case manager to serve people in the Choice Neighborhood, which include Allendale, Ledbetter Heights, and West Edge. The Choice Neighborhood Implementation Grant is a HUD funded program that focuses on revitalization of these neighborhoods that includes new affordable housing, neighborhood improvements, and services. These services include workforce development, healthcare, and education. VOA is the lead agency focusing on delivering these services. Our case management staff will develop, in collaboration with the client, a comprehensive service plan with achievable goals. Case management provides advocacy in obtaining services both in-house and in the community. The person who fills this position has a strong working knowledge of how social service systems operate and possesses the empathy required to understand and relate to individuals who work within this field and the young people who participate in it. This is a full time position.


    ESSENTIAL FUNCTIONS:
    • Identify the needs of the client and family during the initial intake process.
    • Complete initial assessment including identifying basic needs, work history, and achievable goals.
    • Provide training in soft skills – how to answer the phone, life skills –transportation needs.
    • Refer and assist with food stamps, childcare assistance programs, clothing, etc.
    • Determine client's workforce needs – job readiness classes, create resume, and job placement.
    • Initiate referrals with CN Partners to assist with educational opportunities, employment, and healthcare services.
    • Work closely with Workforce Coordinator in identifying client progress in job readiness, job retention classes and help them focus on goals.
    • Follow up with other CN Partners for progress reports on referred clients.
    • Help client enroll in classes and complete financial aid packet.
    • Coordinate care through home and office visits throughout the month. Weekly contacts: phone contact, emails, text message, zoom meetings, FACETIME, and office and home visits.
    • Maintain all case notes, intakes, assessments, services entries, and updates in database as needed.
    • Provide accurate, complete and timely data, and reports to program director as needed.
    • Connect clients with appropriate behavioral health support.
    • Referrals to community resources for service coordination residents
    • Refer to the Financial Empowerment Center for education on banking.
    • Meet weekly with CN staff to discuss client needs.
    • Participate in required agency training.
    • Attend CN meetings and all other CN activities necessary for meeting CN outcomes.
    • Contact the program director for needed support and direction.
    • Complete other assignments as directed by the Program Director.



    REQUIREMENTS:
    • Possess a passion for people who are underserved.
    • Requires a bachelor's degree in social work or related field.
    • A minimum of three years’ experience working with families in need of community and social services.
    • Ability to be sensitive and responsive to diversity, equity and inclusion.
    • Ability to communicate effectively with clients, co-workers, volunteers, and supervisor.
    • The ideal candidate has worked with coordinating care. Medical experience preferred.

    PREFERRED QUALIFICATIONS:
    • Bachelor's degree in Social Work or related field or a minimum of three years of case management experience with an emphasis in working with families.
    • Ability to be sensitive and responsive to diversity, equity and inclusion;
    • Demonstrate honesty and authenticity; Ability to set limits and boundaries that maintain the helping role and assist others with setting healthy boundaries;
    • Ability to communicate effectively with CNI partners, co-workers, volunteers, and supervisors. The ideal candidate has worked with coordinating care. Medical experience preferred.
    • Ability to recognize Trauma and ACE’s



    OTHER REQUIREMENTS:
    • Must have a valid Driver’s License with good driving record
    • Liability insurance that meets minimal requirements
    • Vehicle must display current safety inspection sticker
    • Criminal background will be checked
    • Must have a cell phone
