Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. SUMMARY: Promoting Adolescent Strengths (PAS) Case Manager provides services to adolescents and their parents/caretakers, who are consumers in the Department of Children and Families. These families reside in the Springfield, Van Wart, and Holyoke Are Office Catchment areas. The Case Manager provides intensive home visiting services and delivers an array of interventions, which are identified by the funding source (Department of Children & Families (DCF).
ESSENTIAL DUTIES AND RESPONSIBILITIES:

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION AND/OR EXPERIENCE: Bachelor’s degree preferred; Associate’s degree and two years of college in an appropriate field accepted. Knowledge of adolescent development, family systems, strength-based practice, child welfare and local social service network.
LANGUAGE SKILLS: Bi-lingual (English/Spanish) and Bi-Cultural is required.Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers, parents and employees of organization.
REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
MENTAL/VISUAL DEMANDS: Flow of work and character of duties involve normal mental and visual attention much or all of the time. Specific vision abilities required by this position includes close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Eye-hand coordination required less than 15% of the time.
