Job responsibilities include managing a patient caseload that includes both home health and hospice patients. Effectively communicating with patients, families, physicians and other disciplines to ensure a comprehensive approach to care. Participate in team meetings, accurately document patient progress and changes in patient's conditions. Submit required documentation in a complete and timely manner. Function as a team player within the interdisciplinary group of healthcare professionals.

This position requires on-call, is primarily Monday through Friday daylight hours, with limited on-call hours. Compensation will include additional reimbursement for on-call.

The successful candidate will have at least one year of previous experience in home health or hospice.
