Summary
Participates as a member of a multi-disciplinary team in providing case management/discharge planning interventions to all patients with mental health and substance abuse issues within the BHS system. Provides interventions to mobilize and strengthen internal and external resources to enhance patient's ability to cope with their mental illness and to promote a safe and timely discharge.
