Case Manager (Non-RN) jobs in Concord, NH

PURPOSE

• The RN Case Manager will be responsible for providing care and services for hospice patients where they reside. 

KEY RESPONSIBILITIES

• 1. Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license. Admissions and On-call experience also preferred
  2. Performs assessments of patient/family physical, emotional, psychosocial, spiritual, and culture needs to ensure and coordinate the delivery of comprehensive, comfort-focused services through the development of the Hospice patient plan of care. 
  3. Provides a full range of professional nursing care to patients via phone or in their homes, under the medical direction of a licensed physician. Maintains accurate, up-to-date records of patients and care given.
  4. Documents all patient/family interactions in the medical record per agency policies/protocols. 
  5. Works well within a team environment to collaborate with nurses, social workers, chaplains, volunteers, and LNA’s.
  6. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  7. Regularly reevaluates patients' nursing needs and informs physician and other personnel of any changes in patient's condition or needs. 
  8. Responds to overnight calls for designated needs (secondary triage; initial triage handled by third-party administrator)
  9. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
  10. Represents VNH in a positive, proactive manner while participating in continuing education programs, conferences and workshops.
  11. Meets Agency's productivity expectations, census permitting.
  12. Continues professional development by participating in continuing education programs, conferences, and workshops.
  13. Incorporates infection control protocols with professional practice.
  14. All other duties, as assigned. 

MINIMUM QUALIFICATIONS

• • Graduate of an NLN accredited school of Nursing. Baccalaureate Degree preferred.  
  • Certified in Hospice/Palliative Care Nursing desirable.
  • Minimum two years’ general experience in a supervised nursing setting recommended. Hospice/Homecare experience preferred.

REQUIRED LICENSE/CERTIFICATIONS/REGISTRATIONS SKILLS (Tracked at the department level)
