Responsibilities:

- Collaborate with healthcare professionals to coordinate and manage patient care
- Monitor patients' progress and make necessary adjustments to the care plan
- Provide education and support to patients and their families regarding their healthcare needs
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information
- Facilitate discharge planning and coordinate post-discharge care services
- Collaborate with behavioral health professionals to address mental health needs of patients
- Utilize critical thinking skills to assess, prioritize, and respond to patient needs in a fast-paced hospital setting
- Perform diagnostic evaluations and provide appropriate interventions as needed
- Provide nursing care in various settings including critical care, acute care, and pediatrics

Experience:
- Valid Registered Nurse (RN) license
- Previous experience as a case manager or in a similar role is preferred
- Strong knowledge of healthcare regulations, including HIPAA
- Excellent communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
- Strong organizational skills with attention to detail
