Pleasant Valley Hospital is seeking a full-time RN Case Manager. This position monitors a multifaceted process which encompasses case management, social services and discharge planning of patients in order to facilitate achievement of quality outcomes. Assesses patient needs for case management intervention through data gathering, treatment plans, resource utilization and cost management; coordinates patients' care throughout the continuum of care.
Requirements:
Current WV RN license. Graduate of an accredited school of nursing. Must be able to work all shifts. Position primarily is located in the Emergency Department. Previous Case Management experience preferred.
