Job Overview:
We are seeking a dedicated and passionate Case Manager to join our team. As a Case Manager, you will be responsible for providing individualized case management to people living with HIV in Indiana.

Education/Experience:
Bachelor’s degree preferred. Equivalent experience will be considered.
2 years' experience in case management preferred.
A combination of lived expertise, work experience, and/or certification as defined below may be considered in lieu of degree.
Lived Expertise
Living with, in recovery from, or caring for others experiencing, viral hepatitis, HIV, substance use disorder, homelessness, trauma, mental illness, etc.
Experience accessing healthcare as a member of communities that have been traditionally excluded from access including Black, Indigenous, and other People of Color (BIPOC); lesbian, gay, bisexual, transgender, queer/questioning, intersex (LGBTQIA ); physical ability, or national origin, people who have experienced homelessness, or anyone with a history of criminal justice involvement.
Language Ability:
Ability to read and speak English required.
Other Skills:
Proficiency with Word, PowerPoint Excel, and Outlook, and ability to learn additional programs, required.
Dependable transportation required; position will entail weekly travel in Central Indiana, as well as travel throughout the state of Indiana. In some instances, overnight stays may be necessary. Out of state travel with overnight stays may be required for conferences, training, and continuing education opportunities.
