Compensation will be determined based on years’ of experience, applicable certifications and by the number of visits completed each week.

QUALIFICATIONS for the Registered Nurse (RN) Case Manager

• Graduate of state-approved school with course in pharmacology
• Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.

Job Functions:

• Act as primary liaison between physician, patient/family and home health team.
• Provide direct patient care services to patients and supportive care to the patients’ families.
• Manage patient case load.
• Building a trusting and compassionate relationship with your patients and their families.
• Empower patients’ caregivers by educating them on care for their loved one.
• Manage and collaborate with an interdisciplinary team.
• Evaluate patient and family needs, and assist in the preparation and execution of the plan of care for each patient.
• Maintain all assigned patient clinical records.
• Participate in company quality assessment and performance improvement programs.
