Case Manager (Non-RN) jobs in Las Vegas, NV

Utah Case Management is looking to hire a Registered Nurse to join our case management team. The position is very flexible allowing you to set your own hours and schedule. The position is mostly remote, but would require the nurse to travel within their area to visit clients. This is a less than part time position that is located in Washington County area.

Responsibilities:

• The main responsibility would involve completing medication management reviews for clients: Obtaining a list of the client's meds, assess medications, and complete medication management review paperwork.
• Assessing clients' health condition and identifying supports needed for their health and safety.
• Managing clients' services: evaluating the services the clients are receiving to make sure they fit the client's health needs, coordinating services for clients, etc.
• Reviewing clients' care, services, and medication on a regularly and when a client has had a change in health status.
• Documentation: completing, updating, submitting, and compiling necessary documentation for clients.
• Communicating with the social work case manager, facility staff, and service providers for coordinating clients' care.

Qualifications

• Utah Resident
• Current Registered Nurse license in the State of Utah
• Good at maintaining relationships with clients
• Detail oriented
• Able to work independently and with a team

Job Type: Part-time

Pay: $20.00 - $30.00 per hour
