Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. Qualifications for Nurse Case Manager:
Current unencumbered State of New Hampshire RN/LPN license
Knowledge of Medicare, Medicaid, PPS, PDPM, MDS and third-party payment
Knowledge of regulatory agencies/licensure standards
Must have minimum 5 years’ experience working in geriatrics with subacute experience
Must have strong clinical background
Case management experience strongly preferred
Responsibilities for Nurse Case Manager:
Monitoring level of HMO patients on admission and throughout stay
Obtaining prior approval for all HMO patients throughout the facility for transportation, therapy, etc.
Tracking HMO and Medicare days
Obtain pre-authorization as needed for special services and equipment
Ensuring sound clinical and financial outcomes
Attending family meetings
