Case Manager, Bilingual - Employment Services
JOB SUMMARY: In support of the Access Mission and Strategic Commitments, the Case Manager, Bilingual - Employment Services provides case management and works with customers to develop individualized service plans that outline goals toward skill building and self-reliance with an emphasis on those who need employment and job readiness supports,coordinates the Access to Employment program, ensures proper data entry, and addresses social determinants of health-related to services provided.
ESSENTIAL FUNCTIONS and DUTIES:
- Case management
- Complete a pre-assessment that may lead to a full assessment through the process of customer interview and universal intake
- Collect customer demographic information and income source (if any) during the intake process
- Provide crisis intervention support with non-judgment and sensitivity
- Conduct a competent problem analysis; select services or interventions that are appropriate to help customers attain the desired outcome, or advocate a suitable referral system to sustain the customer through a crisis
- Maintain confidentiality to protect customer records
- Provide referrals and follow-up to appropriate programs and services provided by Access or other community providers that meet the customers' needs
- Provide counseling related to; and monitoring, developing, securing, and coordinating services
- Travel may be required between both sites (Willimantic and Danielson)
- Work with customers to develop individualized service plans that outline goals toward skill building and self-reliance emphasis on those who need employment and job readiness support
- Identify internal and external resources that would enhance an individual or family’s capability to meet goals
- Enhance job readiness through service provision and individualized coaching
- Advocate for customers to area service providers to assist them in moving forward on the way to meeting their goals
- Adhere to the Principles of ROMA-based Case Management set by Community Action Agencies in Connecticut to:
- Provide supportive services that give customers/clients a solid foundation to reach their goals
- Provide information to customers that will enable them to utilize better community services, including health, social, employment, and other beneficial services
- Assist customers in gaining the skills and income needed to achieve and maintain an improved level of self-sufficiency
- Empower and support customers/clients to become their advocates
- Coordinates Access to Employment (A2E)
- Recruit and retain A2E participants
- Conduct outreach and promote the program
- Develop, maintain, and support relationships with partner agencies that send participants
- Work with the consultant team to confirm the curriculum
- Schedule all A2E meetings with consultant staff
- Maintain A2E Drive and social media platforms
- Support current students and graduates outside of scheduled class time
- Ensure linkages for A2E graduates with internal job creation programs
- Address social determinants of health related to services provided
- Provide services within the program services that will have the potential to improve customer health outcomes
- Other duties as assigned
- Support the Director - Case Management Services as needed
- Participation and completion of training requirements related to Community Health Worker Certification
- Other tasks reasonably related to the position that helps drive our Vision, fulfill our Mission, and abide by our Organization’s Values
QUALIFICATIONS:
- Bachelor’s degree in a related field*
- Effectively works independently in a fast-paced environment
- Works well with a diverse group of managers
- Problem solver, solution generator
- Understands and utilizes basic budgeting principles and skills
- Good computer skills with working knowledge of word processing and spreadsheet applications
- Bilingual in Spanish and English required
- Complete Community Health Worker Training within four months of training availability

PHYSICAL DEMANDS:
- Light Work - Lifting no more than 20 pounds occasionally and up to 10 lbs frequently
- Frequently required to continuously stand, walk, sit, and consistently use either hand or foot controls
- The person in this position often communicates with individuals outside the organization
- Must be able to hear and vocalize effectively to exchange accurate information
- This position may require the ability to transport self off-site into environments the agency can not control
- The employee will occasionally be required to read and comprehend complex grant documents, program plans, and agency policies and procedures

Base Rate: $19.16 per hour $1.00 per hour for bilingual designation
Job Type: Full-time
Pay: $22.16 per hour
Expected hours: 35 per week
