Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. Responsibilities:
- Completes an initial, comprehensive, and ongoing assessment of patients and family to determine hospice needs.
-Assesses and evaluates :
A. Writing and initiating plan of care
B. Regularly re-evaluating patient and family/caregiver needs.
Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given.
Experience:
- Hospice experience required.
Requirements:
- Valid Registered Nurse license in California
- Bachelor's degree in Nursing (BSN) is preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Strong critical thinking and problem-solving abilities
- Ability to prioritize and manage multiple cases simultaneously
