RN Hospice Case Manager (7 on 7 off Day Schedule)

(full-time, direct-hire position with excellent full benefits, bonus and compensation packages!)

Responsibilities:

• Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
• Admissions of patient for services and development of the initial patient/family Plan of Care
• Uses health assessment data to determine nursing diagnosis
• Coordination of services with the patient, family, and hospice interdisciplinary team members

Requirements:

• Valid Registered Nurse (RN) license
• RN Diploma, Associates or B.S. degree in nursing
• Minimum 1 year of clinical experience, home care or hospice preferred
• Excellent observation, problem solving and communication skills
• Compassion and integrity

Job Type: Full-time

Pay: $95,000.00 - $105,000.00 per year
