Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. Description
The RN (Registered Nurse) Case Manager provides case management and nursing services to Hudson Valley Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the Agency. Register Nurse (RN) Case Managers work in various settings: Nursing Homes, Home Care and In-Patient (throughout various hospitals). As an RN Case Manager, you are assigned to a specific team to work within; Home Care, Nursing Home or In-Patient. Each RN Case Manager provides pain and symptom management, recommendations and collaborates with the other Interdisciplinary Team members.

Hours: Monday-Friday (8:30am-4:30pm); on call rotation obligation(3-4 times per years)
FLSA: Exempt
Pay: $80,000-90,000 per year
