Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience.     Job Requirements:     Bachelor’s degree in a field of science required, Master’s degree in Social Work preferred. One (1) to three (3) years of general clinical case management experience required. Knowledge of Ryan White Part B Service Standards. Knowledge in chemical dependency, mental health, housing, and homeless service systems required. Skills include excellent oral and written communications including telephone, organizational, and personal computer including word processing and Microsoft office. Must demonstrate skill and knowledge in case management, client assessment, engagement, and group exercises.
