Our client is looking for a Hospice RN Case Manager to join this amazing Team in the charming city of Beloit, WI. If you are a Registered Nurse with Hospice experience this may be the perfect fit for YOU!

RESPONSIBILITIES:

• coordination of services provided by the Interdisciplinary Team
• assuring that the patient and the family needs are met
• case is provided consistent with federal and state regulations
• follow Regional Hospice policies and procedures, and hospice standard of practice

SALARY: based on experience

REQUIREMENTS:

• Graduate of NLS school of nursing
• BSN preferred
• Current RN license in WI and IL, or be able to obtain both
• Valid DL
• reliable transportation
