Job Scope
(Why does the position exist?) To assist Persons Living with HIV/AIDS (PLWH/A) in attaining and maintaining a maximum level of health and independent functioning through the coordination of and facilitated access to resources, thereby maintaining and improving health outcomes and quality of life while upholding the mission of the agency. Thus, Case Manager (Non-Medical) includes both General Case Management and Psychosocial Case Management.
Job Duties and Tasks
Description of Duties/Tasks Required to Accomplish Essential Functions 1. Resources, Education & Training :
Attend staff meetings, case management department meetings, in-service training
programs, and case conference meetings
Assist with implementation of quality management/improvement and evaluation
Activities
2. Intake and Assessments:
Identify appropriate clients for HIV/AIDS case management through eligibility screening and intake; maintain client eligibility requirements as per contract
Conduct assessment with clients to assess and evaluate client needs
Ensure timely entry of services delivered into appropriate database.
3. Service Plan:
In collaboration with client, develop a comprehensive client-centered service plan to address needs and goals
Ensure timely entry of services delivered into appropriate database.
4. Monitoring and Follow-up:
Contact clients on a monthly basis, or more as needs arise or contract requirements
Maintain thorough, current, and confidential records of each client, as well as ongoing service plan
Maintain accurate records of food and transportation services provided
Ensure timely entre of services delivered into database
5. Advocacy:
Assist clients and/or families with the completion of necessary applications for community services and public benefits
Provide appropriate service referrals and resources for the client and advocate on the client’s behalf for accessibility of these services and work with client to address barriers to access
Coordinate and network with other agency/community resources to ensure provision of needed services and their delivery
Assist clients in problem-solving as requested, providing counsel and advocacy as needed to encourage clients to access available medical care, food programs, emotional support services, legal assistance, and other services
6. Other
Provide guidance to assigned interns and/or volunteers in the provision of service
Delivery
Facilitate and/or organize client support group including educational seminar and/o
events
Participate in community meetings and represent agency at community events
Prepare monthly/quarterly/annual service reports of case management, food,
Transportation and other reports as needed; reconcile food and transportation data
entry with delivery; reconcile service units against productivity rate
Fully committed to quality assurance and cultural competency
Ensure timely entry of services delivered into appropriate database.
Heavy data entry from initial recruitment of a client to case closing of a client.
Mandated 24 hours entry of a progress note.
7 Other Duties:
Time and Labor: Employees must record their time accurately and in accordance to time worked. All time must be recorded within the Stratustime System.

Years/Type of Exp:
Minimum three years of relevant experience; experience working with persons of different cultural and socio-economic backgrounds
Education:
Bachelor’s degree in health or human related field preferred or three years relevant and documented experience in a similar field with an AA degree in health or human-services field.
