Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. Looking to add a Case Manager to our amazing team! Looking for 6 or more years of experience with IDD or a licensed social worker and/or a bachelor's degree in social work, Psychology, Counseling, Nursing, Special Education or a closely related field. You will be working with Individuals with Developmental Disabilities. You will make phone calls, meetings and have monthly visits to address any concerns the Individuals may have. You will be responsible for following up on appointments and any issues that may arise. A Case Manager is required to ensure the individual is receiving the requested services and each of their needs are being met. You will be required to ensure the individuals health and safety.
