Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. Willis-Knighton Hospice, part of the Home Care Department, is seeking a Registered Nurse to join our Hospice team.
The ideal candidate will have a strong work ethic, excellent attendance, the ability to communicate effectively, and the passion to care for those who are at end of life. The Hospice nurse works with a caring, dedicated team to provide care and comfort which encompasses physical, emotional, social and spiritual needs.
A minimum of one year recent experience as a Registered Nurse in an acute care setting is required. Proficient computer skills are necessary as patient documentation is done electronically.
