The Case Manager is responsible for assessing, planning, coordinating, monitoring, and evaluating the options and services to meet an individual's health care needs across the continuum. The Case Manager assists the physicians and staff to determine the appropriate level of care and post-acute care needs.
This position reports to the Director, Case Management and Social Work and operates within established organizational and departmental policies and procedures.
