Registered Nurse Case Manager
Overview:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will play a vital role in providing comprehensive patient care and coordinating services for individuals in need. This position requires expertise in pediatrics, patient assessment, medical terminology, and diagnostic evaluation. - Conduct patient assessments to determine individual needs and develop appropriate care plans
- Coordinate and collaborate with healthcare professionals to ensure the delivery of high-quality patient care
- Monitor patient progress and make necessary adjustments to treatment plans
- Provide education and support to patients and their families regarding medical conditions, treatment options, and available resources
- Maintain accurate and up-to-date medical records
- Perform routine patient care tasks such as taking vital signs, administering medications, and assisting with activities of daily living
- Advocate for patients' rights and ensure their needs are met within the healthcare system
- Collaborate with primary care providers to ensure continuity of care
- Participate in interdisciplinary team meetings to discuss patient progress and develop effective care strategiesQualifications:
- Valid Registered Nurse (RN) license in Ohio
- Strong assessment skills to identify patient needs accurately
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to work independently as well as part of a collaborative team

Note: This is a paid position, and all applicants must be eligible for compensation. Internship positions are not available.
Job Types: Full-time, Part-time
Pay: $30.00 - $50.00 per hour
Schedule:
- Monday to Friday
- Weekends as needed
