Job Overview:

• Provide comprehensive social work services to program participants. This includes but is not limited to psychosocial evaluation, assessment for admission, patient’s rights and advocacy, social casework, group and family counseling, as appropriate. Function as a key member of the interdisciplinary team.

What You Will Do:

• Case Management, care plan, maintain and manage resident chart per regulations.
• Integrates the social work treatment plan into the overall plan of care developed by the interdisciplinary team.
• Review inspection reports with the Administrator retrospectively to institute a plan of correction as required. Monitors results.
• Assist in the development and implementation of social work policies and procedures in collaboration with other team members and the Quality Assurance lead.

What You Will Need:

• 1 years of RN Exeperience preferably in Assisted Living or LTC
• Case Management Experience within Medicaid (Preferred) ALP and UAS Experience preferred but not required.
• Registered Nurse licensed in the state of NY
