Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. We are growing and looking for competent, caring Nurses to join our professional and compassionate team. This is a full time position Monday - Friday, 8am/5pm. Will also rotate with the other CM's through a Back Up On Call schedule (paid at an additional rate).
Must be able to work independently and also as a vital part of our IDT Team as we provide care for our patients. Team consists of an RN, LPN, Chaplain, Social Worker and Aides.
