    DEPARTMENT: Nursing

    POSITION: Registered Nurse - Case Manager

    SUPERVISOR: Vice President of Assisted Living Services

    SALARY: $62,400 - $72,150

    SHIFT: 8:00AM - 4:00PM


    QUALIFICATIONS:

    Graduate of an accredited school for registered nurse licensure.

    Currently licensed in New York State to practice nursing as a registered nurse.

    Must possess good organizational, decision making, and effective communication skills in addition to leadership ability.


    DUTIES:

    • Conducts initial assessment of assisted living resident to determine appropriateness for residency at Cedarbrook based on admission standards and regulations.
    • Works in conjunction with the resident's primary care physician and resident representative in regard to the following:
    Admission to Cedarbrook;
    • Development of care plan and coordination of care for the resident;
    • Proper discharge planning for the resident as needed;
    • Completion of all Office of the Aging paperwork in regard to initial admission, hospital stays, transfers to inpatient rehab, and discharges, as appropriate.
    • Assures assessment of resident's status, effectiveness of intervention, and the ability to meet goals/outcomes as necessary, at least every six months.
    • Performs assigned nursing procedures according to the regulations as well as for the comfort and well-being of the residents.
    • Responsible for the timely and accurate administration and documentation of physician ordered medication.
    • Obtains and transcribes physician orders according to facility policy and procedure and assures proper interpretation and execution of same.
    • Observes and records in a timely manner any resident findings and care in the medical record and reports all changes in a resident's condition to physician and family representative as appropriate.
    • Provides resident teaching, wellness counseling, health promotion, and disease prevention.
    • Performs and provides for orientation, teaching, and supervision of Home Health Aides including quarterly review of medication course and supervised visits and maintains all paperwork associated herewith.
    • Conducts in-services to nurses and HHA's routinely and as assigned by supervisor.
    • Participates in disciplinary action and counseling of staff as needed, as well as conducting written performance evaluations of nurses and HHA's as assigned.
    • Reviews medication regime on a monthly basis with those residents who require only supervision of self-medication and provides resident teaching as needed.
    • Assures that discontinued/expired medications are disposed of according to facility policy.
    • Performs quality assurance audits as directed.
    • Completes and timely posts nursing schedule.
    • Maintains documentation of shift occurrences to promote thorough reporting of resident activity to oncoming shift.
    • Makes appropriate medical decisions within scope of licensure.
    • Works under limited supervision.
    • Provides 24-hour on-call RN services.
    • Updates nursing policies and procedures as changes occur.
    • Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws. Adheres to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties.
    • Performs any other duties as assigned by the Vice President of Assisted Living Services.


    Requirements:

    The case manager must have the following education and work experience:

    (i) be a currently registered professional nurse with one year of full-time experience; and

    (ii) a master's degree in social work from an accredited college or university; or

    (iii) a bachelor's degree from an accredited college or university with major work in human services or service delivery and one year of full-time experience in the provision of services to a dependent adult population; or

    (iv) an associate degree from an accredited college or university with major work in human resources or service delivery and three years of full-time experience in the provision of services to a dependent adult population.
