Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
Provide services that result in timely and safe achievement of company and state outcomes relevant to the position.
Provide direct practice and case management services to include child visits and monthly supervision of the home.
Help prepare children for adoption and make referrals to the Adoption Therapist. Update the child’s social history, child’s plan of service, attend court hearings, complete the sibling split request, and review the child’s file with the selected family.
Assist in matching adoptive families with children eligible for adoption.
Support adoptive homes and ensure all required trainings, documentation, and background checks are completed.
Provide training and preparation for adoptive families.
Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
Back up on-call to the on-call department.
