POSITION SUMMARY:

The Practice Physician provides medical evaluation, treatment and education to primary care patients under the direction of the Medical Director. The focus of medical Home, Patient Centered Care is to provide treatment and preventative care to clients and families from infancy to early adulthood.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Assess each patient fully including physical, mental and emotional assessment, family history, medical history, allergies, socio-economic level and cultural practices. Provide age–appropriate and culturally-appropriate care to patients and families; identify further need for teaching
• Order in-office procedures, lab testing and other off-site tests as necessary
• Identify the need for patient and family teaching and teach or delegate teaching to other care team members as appropriate
• Coordinate with nursing follow up on problems identified at the visit/alert Nurse Coordinators to high-risk patients
• Use evident based assessment and treatment to evaluate patients and monitor follow up
• Act as clinical resource to other providers, nursing and other members of the care team
• Perform physical examinations within prescribed protocols set forth by the practice Medical Director and document these assessments in a timely and comprehensive manner
• Refer to outside resources when necessary such as physical therapy, speech therapy, or psychopharmacology
• Maintain pediatric-centered continuing education and training
• Adhere to the general policies and procedures as outlined by the practice
• Ability to meet necessary physical demands of the position

Requirements

OTHER QUALIFICATIONS:

• Excellent written and verbal communication, excellent organization
• Ability to work independently as well as function as part of the care team
• Ability to handle sensitive medical information/HIPAA compliance
• Working knowledge of EMR, Microsoft office and email

EDUCATION AND/OR EXPERIENCE:

Pediatric M.D. with a MA medical license

BC/BE
