Case Manager - Physician Practice jobs in Orlando, FL

A busy family practice clinic that provides treatment in primary care is looking for a provider to help with the growth of the practice in multiple locations (Clermont/Ocoee/Four Corners).

Duties and Responsibilities include but are not limited to:

Evaluate sick and urgent care patients, transition of care, physicals, and also educate patients on maintaining proper health

Evaluating, making recommendations, co-managing, and treating patient's medical needs

Participate in an interdisciplinary team providing care and coordination of our patients ensuring the highest level of care

Any additional qualifications are not required however are favorable:

-Spanish Fluency
