Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. As a Case Manager you will be responsible for verifying managed care benefits, obtaining authorizations and managing concurrent insurance reviews. You will work closely with the facility IDT teams and maintain member caseloads. The Case Manager collaborates with the Social Services Director at each facility with regards to discharge planning. Your work will ensure our residents receive the high standard of care they have grown to expect at our facility.

As a minimum, a high school diploma or equivalent and two (2) years of experience in a Skilled Nursing Facility required. Prior experience in Case Management, Admissions, Clinical Services or Social Services preferred. Point Click Care experience a plus.
