Case Manager, Sr. jobs in Dothan, AL

Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for assisting eligible clients with basic needs and services. This is a rewarding opportunity to make a positive impact on the lives of others in the Wiregrass community.

Duties:

• Schedule appointments for clients who walk-in or call for assistance.
• Determine which services and assistance are necessary based on in-person interview.
• Develop a plan of action with the client to improve self sufficiency and well being.
• Assist clients within the scope of the Direct Aid/Emergency Assistance programs.
• Coordinate monthly home visits for clients pursuant to individual circumstances.
• Supervise and monitor client progress, ensuring that goals are being met.
• Advocate for clients' rights and best interests.
• Create, file, and maintain case reports.
• Bag food for clients bi-weekly and as needed.
• Perform other duties as assigned by Director.

Requirements:

• Bachelor's degree in social work or related social services field.
• Previous experience in Case Management.
• Compassionate and empathetic approach to working with individuals and families.
• Strong organizational skills with the ability to prioritize tasks effectively.
• Excellent oral and written communication skills.
• Ability to work independently as well as part of a team.
• Valid Alabama Driver's License and use of personal insured vehicle.
• Candidate must successfully complete Archdiocesan Child Protection Training and background check prior to employment.

Pay: From $16.00 per hour

Expected hours: 35 per week
