Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. Mountain Comprehensive Care Center is hiring for a Targeted Case Manager based in Paducah and will serve the McCracken and surrounding counties .
PURPOSE: To assist individuals with severe mental illness (SMI), severe emotional disorder (SED), substance use disorder (SUD), or chronic and complex physical health with behavioral (BHPH) in negotiating for needed services and resources in the community and mental health settings.
MINIMUM QUALIFICATIONS: BA or BS in any field of Behavioral Science or Human Services from an accredited institution. Minimum of one year experience in case management or working with the severely mentally ill and one year experience working with adolescents. A Master’s degree in a human services field may be substituted for the one year of experience. Must complete the case management certification program offered and approved by the DMHMR within 6 months of employment. Must have a valid Kentucky motor vehicle operator’s license.
SPECIAL KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of and personal experience with public mental health services. Strong reading comprehension, verbal and written communication skills. Demonstrates the ability to communicate with others in a professional manner.
