Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. POSITION SUMMARY: Responsible for coordinating the provision of services on behalf of persons with Intellectual/ Developmental Disabilities, in gaining access to needed services, i.e., social, medical, educational, vocational.
QUALIFICATIONS: Bachelor Degree in psychology, social work, or related field in human services; preference for work experience with IDD population
SKILL SETS: Possesses superior organizational, oral, and written communication skills. Must demonstrate a high level of familiarity with Microsoft Office and exceptional customer service skills. Must be dependable, interact effectively with the public and staff, adaptability-able to deal with frequent change, delays or unexpected events, strong sense of commitment.
PHYSICAL AND ENVIORMENTAL DEMANDS: Frequent talking and active listening when communicating to individuals, families, guardians, and staff. Possible exposure to communicable diseases, possible exposure to blood borne pathogens. Visual acuity necessary for appraising the appearance of individuals and their environment. Conduct home visits with individuals which may cause exposure to risk of unsanitary conditions, abuse and violence. Work is light, standing and sitting, occasional stooping. Ability to handle stress effectively and make decisions accordingly.
