Case Manager, Sr. jobs in Mesa, AZ

Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     Who We Need

    We’re looking for someone with the following skills, experience and credentials:       
    • Works collaboratively with the integrated care team to engage, educate, communicate, and coordinate care with service participants, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the Integrated Service Plan are implemented.
    • Develops and maintains and manages the Integrated Service Plan (ISP) within required time-frame.
    • Participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP).
    • Implements services to members in accordance with their plan and provides ongoing care and reviews progress.
    • Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.  

    Job requirements       
    • High School degree required, and minimum four (4) years of experience in behavioral health and SMI, or
    • Bachelor's degree in a related field with no experience.
    • AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements.
