Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. JOB DESCRIPTION: MHC MANAGER/MHC/CASE MANAGER
Essential Functions
Supervisory Responsibility
This position manages all employees under the case managers scope, which may include.
o Responsible for performance management of direct care staff.
o Ensure all CTL policies and procedures are being enforced.
o Coaching and Development.
o Training and Education of Direct Care Staff.
o Create and administer weekly schedules and ensures shifts are properly covered on a daily basis.

Work Environment
This job operates in the community, specifically in client’s homes, 2-3 days per week, in the office 1-2 times per week and provides a work from home flex day 1-2 times per month. (Due to Covid-19 restrictions, in home visits are based solely on the client’s needs). This role uses standard office equipment 1-2 times per week.
Travel
Travel is required throughout the region. A company vehicle will be provided along with training from the Worker’s Compensation Trust.
Physical Demands
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; talk and hear. The employee must occasionally lift or move office products and supplies, up to 20 pounds.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday, 8 a.m. to 4 p.m. (hours can be flexed). The position requires on-call shifts.
Required Education and Experience
Master’s Degree and or 3-5 years of previous work-related experience.

MHC Manager Duties
Managing MHC managers, holding mandatory monthly MHC meetings, filling in for MHC managers if needed, reviewing potential MHC managers, encouraging, and promoting MHC clients to direct care clients.

Summary/Objective
The MHC Manager/MHC/Case Manager hybrid position is directly responsible for the processes surrounding a client’s growth based upon the individual’s treatment plan. This hybrid position is specifically geared towards managing mental health counselors and direct care staff.
