Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. Loeb House is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive support and assistance to individuals, including veterans, who require guidance as they create housing plans that will enable them to live independently within their community. We are seeking 3 Case Managers for the following shifts
40 hrs. Tue. – Sat. 12 PM -8 PM
40 hrs. Sun - Thu 9 AM to 5 PM
40 hrs. Thursday - Monday 10 PM to 6 AM -Overnight weekends. Off Tuesday and Wednesday
Responsible for obtaining and furnishing housing for recipients rent collection, apartment inspections, budgeting, traveling to each site, and assisting with initial moves and relocations. Advocating and Supporting our residents.
Basic Job Qualifications:
Veteran Status Preferred, High School diploma or equivalent, College courses or degree preferred. Experience with Mentally Ill individuals and/or Mentally Ill individuals with addictions required. Valid, clean NY driver’s license required. This position requires travel throughout Rockland County to various buildings and apartments. Must be able to climb stairs. Must possess intermediate computer knowledge and skills, and be an effective communicator with the ability to create new relationships with recipients, landlords and outside professionals.
