Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     Position Title: Tri-County Community Assistance Case Manager

    Job Summary: Responsible for administration of The Salvation Army's Emergency Assistance Programs in Peoria, Fulton, and Woodford County. Position includes providing resource referrals, screening participants for eligibility with various funding partners, and administering assistance according to agency and funder standards.

    Reports to: Social Services Operations Manager

    Supervises: N/A

    Qualifications:

    Candidates must have a passion for fulfilling the mission of The Salvation Army and pass the pre-employment background inquiry.

    Education:

    Bachelor's degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree.

    Skills:

    • Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
    • Ability to establish policies and procedures; holding others accountable to the same.
    • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
    • Demonstrated awareness of budget resources and cost control, as well as planning and administering budgets.
    • Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications, and case management databases.

    Essential Responsibilities:

    • Conduct outreach and engagement services in the community in order to identify, engage, support and build rapport with participants in the target population.
    • In collaboration with the Canton Director, develop and maintain policies and procedures for the Emergency Assistance and Pathway of Hope Programs in Fulton County.
    • Develops and fosters local relationships with other service/ healthcare/ educational/ municipal entities to ensure effective local support and referral sources.
    • Determine client eligibility and conduct client intakes and assess the level of need; completing all appropriate assessments and paperwork, gathering documentation.
    • Maintain written and electronic case files with all relevant client information along with advocacy, financial services, referrals, payment arrangements, and outcomes.
    • Maintain and submit monthly statistics in a timely order as required for The Salvation Army and other grants received.
    • Collects, compiles, and reports statistical data to the Social Service Operations Manager.
    • Attending staff meetings and community events as necessary.
    • Participate in renewal of all contract and funding plan requests.
    • Other duties as assigned by the Social Service Operations Manager.
    • Maintain positive work atmosphere through appropriate conflict management, courteous demeanor, and professional and appropriate communication practices.
    • Maintain compliance in The Salvation Army's "Safe from Harm" Certification.

    Position Ranking:

    • Regular, Full Time, Non-Exempt

    Physical Demands/ Work Environment:

    This Position is required to do basic business and office work. In order to successfully perform the essential functions of this position, the individual is regularly required to talk and listen, stand, walk, sit and use hands. Individual must be able to regularly lift up to 20 pounds and occasionally lift up to thirty pounds. The use of Office and Computer Equipment is necessary. Vision abilities will require close vision. Work may be performed indoors and occasionally require outdoors to attend meetings. There is low to moderate noise level for this position.
