Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. Responsibilities
Coordinate and provide that care is safe, timely, effective, efficient, equitable, and client-centered
Handle case assignments, draft service plans, review case progress on an ongoing basis.
Help clients achieve wellness and autonomy
Facilitate multiple care aspects (case coordination, information sharing, etc)
Help residents make informed decisions by acting as their advocate regarding their clinical status and treatment options
Develop effective working relations and cooperate with medical team throughout the entire case management process
Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
Record cases information, complete accurately all necessary forms and produce statistical reports
Promote quality and cost-effective interventions and outcomes
Assess and address motivational and psychosocial issues
Adhere to professional standards as outlined by protocols, rules and regulations
Qualifications
•
Achieved a Bachelor's Degree in Human Services, Case Management, Social Work or the equivalent
•
Case Management experience in assisted living or equivalent
•
Experience working with the adult or geriatric population
•
Good communication, computer and people skills
•
Able to establish working relationships with resident and families
•
Be able to work as part of a team
•
You must be legally authorized to work in the United States
•
