Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature. Position Summary:Person in this position will work in collaboration with a counselor to provide case management services to patients/clients of WestCare who have been diagnosed with a substance abuse disorder, a mental illness or both. Essential Job functions are listed below. Other duties may be assigned, as needed.
Essential Job Functions:- Assess Resource needs of patients.
- Educate and refer patients to community agencies and businesses that appropriately address the patients’ needs for food, clothing, shelter, employment transportation, etc.
- Assist patients with establishing or re-establishing support systems within their community.
- Act as advocate on behalf of the patient to improve access to needed services.
- Participate in clinical staffing decision regarding the patients’ current strengths and resources and their need for additional resources.
- Complete documentation to services on provided forms in a timely manner.
- Provide input into the patient’s treatment needs and disposition.
- Assist other WestCare staff as needed in a collaborative manner to promote cooperation and portray a professional image to the community.
- Embrace and embody mission, vision, guiding principles, clinical vision and goals of WestCare Foundation.
- Perform other duties, as assigned.
Essential Qualifications:- Ability to work collaboratively with others in a manner that is pleasant and professional
- Ability to work well in a team environment
- Ability to exercise good judgement and discretion
- Ability to operate a computer and complete documents in Microsoft word program formats or the ability to learn.
- Ability to complete work tasks within scheduled work hours.
- Excellent verbal skills and written communication skills.
- Willing to seek out new work projects and contribute ideas and efforts to the constant improvement of WestCare services.
Education:- Bachelor’s Degree in human services field required
