Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     Location: Avondale Park (Redmond, WA). Initial 2-week training is in-person at a Hopelink worksite.

    Work Schedule: Full Time. Monday – Friday 8:30am – 5pm. After training, hybrid in-person with remote work options available. (3 days onsite and 2 days remote)

    The career opportunity:

    Hopelink provides vital resources to people experiencing financial and housing instability in our community. The Case Manager provides assessment, advocacy, referrals, and other support services to low-income families in shelter, transitional, non-time limited housing programs as well as community-based settings. The primary goal is to help families through their own resilience to make positive life changes and substantial progress towards self-sufficiency, with a focus on increasing income & obtaining and maintaining permanent housing.


    This position offers paid training!


    The ideal candidate will be flexible, compassionate, excited to learn, and is a team player who works well in a collaborative team environment alongside community partners. A combination of on-site and remote options is available for this role after training period is complete and job proficiency is demonstrated. Case Managers may need to provide client coverage at other Hopelink sites.

    • Candidates who are bilingual are encouraged to apply.
    • Experience working with diverse communities, preferably low-income, older adults, people with disabilities, immigrants, refugees, youth, and/or veterans.
    • Two years’ experience in social services or a community-based organization; in housing case management preferred.
    • Knowledge of subsidized housing resources and housing system to address family homelessness preferred.
    • Computer skills required (Word, Excel, Internet). Office 365 and Teams, zoom, SharePoint, databases.
    • Introductory knowledge of human services client assessment methods, principles, and techniques.
    • Able to work independently and on a team.
    • Excellent communication skills (verbal and written).


    Examples of what you will be doing:

    • Manages a moderate size case load of around 15-18 families.
    • Interviews and determines the needs of families and individuals in various life domains.
    • Develops case management case plans in collaboration with families and individuals.
    • Assists families and individuals with identification and achievement of goals including addressing barriers to success.
    • Makes appropriate referrals and coordinates services with community agencies based on clients' needs and interests.
    • Ensure service interactions at all levels are done through a trauma informed perspective, paid training for cutting edge Trauma informed approach training.
    • Provides case management support to families and individuals through regular meetings. Case management support includes but is not limited to assisting families and individuals to monitor progress toward goal achievement, helping clients build and identify strengths and successes, supporting clients to develop and utilize positive problem-solving skills and helping clients identify, develop, and maintain support networks.
    • Provides a safe environment in which families and individuals can learn to predict the positive and negative results of their decisions and actions.
    • Maintains paper and electronic case records and completes monthly statistics and other reports as required.
    • Actively participates in departmental and other agency wide meetings and committees as appropriate.
