Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.   • SUMMARY: Promoting Adolescent Strengths (PAS) Case Manager provides services to adolescents and their parents/caretakers, who are consumers in the Department of Children and Families. These families reside in the Springfield, Van Wart, and Holyoke Are Office Catchment areas. The Case Manager provides intensive home visiting services and delivers an array of interventions, which are identified by the funding source (Department of Children & Families (DCF).

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Work in a dynamic, community-based agency with a positive, enthusiastic spirit that values the family-focused, flexible, unique and complex nature of the New North Citizens’ Council, Inc. (NNCC);
    • Provide home visiting services to adolescents and their families in the home, unless otherwise specified, typically twice per week;
    • Accept case assignments from all three-area offices, regardless of the address of the family;
    • Be rigorously accountable for all hours of work from 8:30am-5:00pm with some flexibility for making home visits at times that are requested by Area Office (e.g. 6pm visit);
    • Maintain an average caseload of 12 families;
    • Provide a wide array of services to the adolescent such as: mentoring, counseling, reproductive health education, job counseling, school and academic guidance, anger management, hobby and recreation identification and enrollment, career planning, sibling relationship, parent relationship, linkage to mental health and other services, future planning and others as identified in the task planning;
    • Knowledgeable in computers and their programs, i.e. Windows, Internet usage, etc.;
    • Document all work in the Family Networks Virtual Gateway database within 24 hours of the activity;
    • Participate in scheduled, face-to-face, supervision for at least one hour per week, every week;
    • Participate in active identification of training needs, and active participation in a professional development plan, which should include the pursuit of formal education;
    • Be adaptable, willing to work extra hours on occasion to help meet agency’s goals and commitments;
    • Actively assists program in setting financial goals and prioritizing effectively;
    • Arrange work logically and effectively, and allocating sufficient time.

    SUPERVISORY RESPONSIBILITIES: This job has no supervisory responsibilities.

    QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

    EDUCATION AND/OR EXPERIENCE: Bachelor’s degree preferred; Associate’s degree and two years of college in an appropriate field accepted. Knowledge of adolescent development, family systems, strength-based practice, child welfare and local social service network.

    LANGUAGE SKILLS: Bi-lingual (English/Spanish) and Bi-Cultural is required.Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers, parents and employees of organization.

    REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

    MENTAL/VISUAL DEMANDS: Flow of work and character of duties involve normal mental and visual attention much or all of the time. Specific vision abilities required by this position includes close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Eye-hand coordination required less than 15% of the time.

    While performing the duties of this job, the employee is regularly required to reach with hands and arms; stand; walk; climb or balance; stoop, kneel, crouch, and/or crawl.
